During your first phone call, my office manager will help you prepare for your initial visit. Prior to your first appointment, you’ll need to download and fill out an assessment questionnaire detailing your medical history. Your information on these forms is essential for my examination and diagnosis, so make sure you bring the completed forms with you when you come. If you have any trouble downloading the forms, call the front office and we’ll send them to you by snail mail or email, whichever you prefer. (We have specific assessment questionnaires for children.) You’ll also need to bring any prescription or over-the-counter pharmaceuticals or nutritional supplements you may be taking, your insurance information, and your most recent laboratory tests. Or, you can have your doctor send the tests to us.

The goal is to assess the nature and causes of your symptoms and condition, make an initial diagnosis, chart a course of treatment, and determine if lab tests are needed to obtain more information. We’ll discuss your medical history and review any prescription or over-the-counter pharmaceuticals, and any nutritional supplements you may be taking. And I may give you a dietary, exercise and lifestyle form to fill out for your follow-up visit.

We’ll review your exercise and lifestyle form, and if needed, I’ll help you develop an effective self-care program as part of your treatment. If we both do our part, you can recover from most conditions and achieve a level of health and vitality your body is designed for.

Our beautiful ground floor office suite is tucked in a tranquil hillside on the east side of Terra Linda in San Rafael, just off Hwy 101. Our office has electromagnetic frequency (EMF) remediation to ensure a minimal WIFI field.
At the front desk, the office manager answers the phone, schedules appointments, processes supplement and herb orders, takes payments, gives you receipts and invoices, electronically bills your insurance (when applicable), and manages the office pharmacy and inventory. Marin Optimal Health is a pay-in-full at the time of your visit practice. We accept cash, check and major credit cards – MasterCard, Visa, Discover and American Express. You may also use your Flexible Spending Account (FSA) or Health Savings Account (HSA) for the visit, but not for supplements. Our fees for services and electronic insurance billing procedures are listed on the Patient Agreements & General Office Policy page. We don’t accept payments directly from insurance carriers. If your insurance covers acupuncture, we may be able to submit claims electronically for you. For this, you’ll need to provide a copy of both sides of your insurance card.
